Thank you for choosing us to care for your dental needs. When you arrive for your first visit, please be prepared to complete all health and insurance information forms that will allow us to begin your dental treatment. If you wish to expedite your visit, please go to the Registration Tab to download those forms and fax them to us in advance. Our fax number is (847) 882-7066.

Your initial exam will last approximately one hour. No patient will be seen without appropriate dental radiographs (X-rays), which may help us in determining a patient’s diagnosis and course of treatment. If you have current X-rays from a previous office, you may request to have them forwarded to us before this visit.

We will perform a comprehensive examination of your teeth and gums, as well as an oral cancer screening exam; your occlusion, or bite, and also any cosmetic issues will be addressed at this time. We may take digital photos or make impressions of your teeth to assist us in diagnosis.

In most cases, we will also clean your teeth during this visit and give oral hygiene instructions to help you care for your own teeth.

These procedures will provide us with information to access your existing dental condition and determine the appropriate treatment for you.
